Step 2: Fill out intake forms

After scheduling your appointment, you'll receive a link via email to complete your intake forms. Kindly fill out these forms at least 3 days before your appointment. This ensures that Dr. Bhakta can thoroughly review and prepare for your visit in advance.

Step 3: Chiropractic Initial Consultation & Adjustment

Please wait in the reception area upon arrival. Dr. Bhakta will personally escort you to begin your first session, which includes a health history review, chiropractic examination, initial adjustment, soft-tissue work, and corrective exercises.
